Role
Summary:
The Assistant Manager with the support of the Program Manager is responsible for planning, coordinating, implementing and reviewing program plans and participating in daily activities for people supported assigned to their caseload.
High School diploma or GED
2+ years experience in a work environment supporting people with intellectual and developmental disabilities
Working knowledge of all Microsoft Office Applications
Must be able to display a high level of professionalism and discretion and understand the sensitive nature of confidential information with superior emotional intelligence.
Valid NYSDL
Provides mentoring, support to DSP staff in providing meaningful and varied activities for the program day. Participates in staffing ratio’s and performs DSP responsibilities including hands on activities and community integration opportunities. Receives, communicates and documents important updates regarding individual’s supported, ensuring that all pertinent information is shared with the individual’s circle of support in a timely manner and that accurate documentation is maintained.
Works with Care Manager to schedule annual and semi-annual meetings with program/care givers to ensure necessary assessments are completed and that meeting space is available. Writes the Staff Action Plan (SAP) or Habilitation Plan in eVero, based on the needs established in the individuals Life Plan while adhering to compliance standards. Completes all of the necessary annual/semi-annual documentation (Personal Page, Supports, etc), prepares packet for permanent files and uploads all information into eVero.
Reviews and distributes finalized SAP and supports to staff via training and tangible binders, distributes to families and Care Managers through eVero unless otherwise specified. Ensures that individual’s information is correct and updates records in eVero, as needed. Organizes, conducts, and writes team meetings as needed. Completes monthly notes in eVero and submits according to OPWDD regulations. Runs eVero reports as required to ensure that daily data is completed within the proper time frame.
May be required to attend and successfully complete AMAP, SCIP and other trainings as determined necessary by the program to meet program needs.
High School or better.
