Essen House Calls is currently seeking Board Certified or Board Eligible Physicians in Internal Medicine or Family Medicine. We have openings in various locations:
Bronx, Brooklyn, Queens, Manhattan, Long Island & Westchester County.
The House Calls Physician provides comprehensive medical care to patients in their homes, focusing on those with limited mobility or access to traditional healthcare settings. This role requires strong clinical skills, compassion, and the ability to work independently while coordinating care with other healthcare professionals. This role will carry the expectation of serving in a supervisory medical director capacity after successful completion of a probationary period.
ResponsibilitiesManage, guide, and supervise interdisciplinary teams.
Conduct thorough evaluations of patients' medical history, physical condition, and medication usage during home visits.
Diagnose medical conditions and develop treatment plans tailored to individual patient needs, including prescribing medications as appropriate.
Collaborate with other healthcare providers, including specialists and nurses, to ensure continuity of care.
Education:
Educate patients and their families about health management, medication adherence, and lifestyle modifications to improve health outcomes.
Maintain accurate and up-to-date medical records for all home visits, including assessments, treatments, and follow-up plans.
Address urgent medical needs as they arise during home visits and provide referrals as necessary.
Manage a schedule of home visits, traveling to patients’ locations within the designated service area.
Assist in arranging additional services for patients, such as physical therapy, lab tests, or home health nursing.
