We have an exciting opportunity for a Primary Care Physician to join our post-acute care team in a skilled nursing facility near Dundee, NY.

This role offers autonomy over your schedule, typically rounding 4???5 days per week.

Key Responsibilities:

Provide comprehensive primary care services in skilled nursing facilities Diagnose and manage chronic diseases, developing and implementing effective care plans Collaborate with clinicians and facility leaders to enhance patient care and services Build strong relationships with patients, families, and staff to ensure quality care Mentor nurse practitioners and conduct clinical case reviews as needed Engage with the post-acute care community to maintain professional networks Contribute to care coordination and intervention planning initiatives

Qualifications:

Minimum 1 year of primary care experience (post-acute care experience a plus) Active/unrestricted New York medical license (or in process) Active/unrestricted DEA/prescriptive authority (or in process) Position Highlights & Benefits:
