Duties DescriptionAs a Medical Specialist, you would actively participate in diagnosing and treating patients within your specialized field and advise and confer with members of other disciplines so that patients may receive the benefits of an overall specialized program of care.
- Assist in the development or revision of medical policy and procedures to ensure the provision of quality care.
- Review lab work, specialty consults, hospital discharges, medical visit summaries, and diagnostic reports to assist the group home RN and treatment team.
- Discuss care with hospital or rehabilitation facility providers to ensure continuity of care between the group home, hospital, rehabilitation setting, and community providers.
- Review and approve discharge summaries prior to accepting an individual back into the group home residence.
- Review medical information for new admissions to a residence to ensure appropriate care.
- Communicate with team members regarding medical issues and participate in team meetings as needed in person or virtually.
- Communicate with family members and advocates to explain medical or treatment issues.
- Provide short‑term urgently needed prescription renewals when a community medical provider may be unavailable.
- Serve as a member of an interdisciplinary treatment team, attend meetings, and serve on various committees and work groups to improve individual services and outcomes.
- Provide after‑hours and weekend on‑call medical telephone consult on a rotating basis.
- Provide coverage of primary medical care for people living in a Comprehensive Adult Transitional Home when the assigned provider is not available.
- Provide medical expertise to Family Care Program and Community Mental Health Nurses as needed.
- Assist with physician coverage as needed in sister regions.
Licensure:
Possession of a license to practice medicine in New York State and certification by a Member Board of the American Board of Medical Specialties (ABMS) in your specialty.
Eligibility:
Not listed as an excluded individual or entity on any Federal or State Medicaid and Medicare exclusion lists. Must obtain and provide a National Provider Identifier (NPI) number and participate in billing through Medicare and Medicaid. Must maintain board certification and license.
