Full-time - 4 days per week with flexible rounding schedule Compensation:
Estimated annual income $231,800-$246,651 with uncapped productivity potential

Position Overview:

Seeking a physician for a facility-based medical director role providing primary care in a post-acute skilled nursing facility. This leadership role includes clinical care, oversight of advanced practice clinicians, and collaboration with facility and organizational leadership.

Job Duties:

Perform primary care rounding in a SNF setting Diagnose and manage chronic conditions and acute illnesses Mentor and supervise nurse practitioners Collaborate with facility staff, interdisciplinary teams, and leadership Conduct case reviews and support quality-of-care initiatives Represent the practice in the post-acute care community Support care coordination and network development

Qualifications:

Minimum 1 year of primary care experience - REQUIRED Post-acute care experience preferred Active NY medical license or in progress - REQUIRED Active DEA and prescriptive authority or in progress - REQUIRED Highlights:
