Responsibilities
- Direct and manage a complete continuum of high-quality care within the care team
- Treat chronic diseases and disorders
- Review patient file/record, including allergies, problems, medications, and immunization status
- Elicit and record information about patient's medical history
- Order or execute various tests, analyses, and diagnostic images to provide information on patient's condition
- Analyze reports and findings of tests and examination and diagnose condition of patient
- Perform patient assessments
- Formulate care plans
Compensation
- The salary range for this role is $226,000 to $366,000 annually based on full time employment

Shift & Schedule
- 4 day work week
Requirements
- Unrestricted licensure in the state of New York at time of employment
- Board Certification or board eligibility in Family Medicine and/or Internal Medicine
- Active and unrestricted DEA License or ability to obtain prior to start
- Required to pass a drug test before beginning employment
