Job Summary

We are seeking a Board‑Certified or Board‑Eligible Endocrinology Physician (MD/DO) to provide comprehensive endocrine care to residents in skilled nursing and long‑term care facilities. The physician will manage complex metabolic and hormonal disorders while working collaboratively with Advanced Practice Providers and facility staff.

Responsibilities
  • Provide on‑site endocrinology consults for SNF/LTC residents
  • Diagnose and manage complex endocrine conditions including diabetes, thyroid disease, osteoporosis, and adrenal disorders
  • Oversee insulin management, endocrine therapies, and treatment optimization
  • Order, interpret, and act on laboratory and diagnostic studies
  • Supervise and collaborate with Nurse Practitioners and interdisciplinary teams
  • Lead quality improvement initiatives focused on metabolic disease outcomes
  • Ensure documentation meets CMS and regulatory requirements
Qualifications
  • MD or DO degree from an accredited medical school
  • Completion of an accredited Internal Medicine or Family Medicine residency
  • Fellowship training in Endocrinology preferred
  • Active and unrestricted New York State medical license
  • DEA registration
  • Board Eligible or Board Certified in Endocrinology
  • Experience in SNF/LTC settings preferred
