We are seeking a mission-driven Primary Care Physician to join a growing outpatient clinic in the Bronx serving underserved populations through an integrated, whole-person care model.
This is an exciting opportunity for a physician who wants to do meaningful work while helping shape clinical workflows, care delivery, and patient outcomes within a developing community health program. While not a leadership role today, there is long-term potential for growth into Medical Director-level influence as the clinic expands.
Responsibilities- Provide comprehensive outpatient primary care services
- Deliver care to underserved and high-need patient populations
- Collaborate with behavioral health, substance use, and care management teams
- Participate in team-based care coordination and interdisciplinary discussions
- Support quality improvement initiatives and patient outcome goals
- Assist with refining clinical workflows and operational processes
- Ensure compliance with clinical standards and best practices
- Build strong therapeutic relationships with diverse patient populations
- MD or DO required
- Active New York medical license or ability to obtain
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Experience working with underserved or vulnerable populations preferred
- Community health, FQHC, or integrated care experience highly valued
- Collaborative and team-oriented mindset
- Passion for mission-driven healthcare and improving access
