Salary: $220,000 - $275,000 per year

This growing outpatient clinic in the Bronx provides whole-person care to vulnerable populations including individuals experiencing homelessness, behavioral health needs, and substance use disorders. We are seeking a mission-driven Primary Care Physician to join a growing outpatient clinic in the Bronx serving underserved populations through an integrated, whole-person care model.

This is an exciting opportunity for a physician who wants to do meaningful work while helping shape clinical workflows, care delivery, and patient outcomes within a developing community health program. While not a leadership role today, there is long-term potential for growth into Medical Director-level influence as the clinic expands.

• Provide comprehensive outpatient primary care services
• Deliver care to underserved and high-need patient populations
• Collaborate with behavioral health, substance use, and care management teams
• Participate in team-based care coordination and interdisciplinary discussions
• Support quality improvement initiatives and patient outcome goals
• Assist with refining clinical workflows and operational processes
• Ensure compliance with clinical standards and best practices
• Build strong therapeutic relationships with diverse patient populations

• MD or DO required
• Active New York medical license or ability to obtain
• Board Certified or Board Eligible in Family Medicine or Internal Medicine
• Experience working with underserved or vulnerable populations preferred
• Community health, FQHC, or integrated care experience highly valued
• Collaborative and team-oriented mindset
• Passion for mission-driven healthcare and improving access
