Join a Multidisciplinary Cardiometabolic Health Program
Health Pro Talent is seeking a Board-Certified or Board-Eligible Cardiologist with a focus on non-invasive cardiology and cardiometabolic health on behalf of a well-established, community-based healthcare organization in Brooklyn. The physician will join a multidisciplinary program alongside a Nephrologist and Endocrinologist.
This is an opportunity to provide coordinated outpatient care for patients with cardiovascular disease and complex cardiometabolic conditions, including diabetes, hypertension, chronic kidney disease, obesity, and lipid disorders.
Responsibilities- Diagnose and manage hypertension, dyslipidemia, coronary artery disease, heart failure, and other non-invasive cardiovascular conditions
- Provide cardiovascular risk assessment and preventive cardiology for patients with diabetes, chronic kidney disease, obesity, and metabolic disease
- Order and interpret electrocardiograms, echocardiograms, stress testing, ambulatory rhythm monitoring, and other non-invasive cardiac studies
- Develop individualized, evidence-based treatment plans
- Manage cardiovascular medications and coordinate treatment of related cardiometabolic conditions
- Collaborate closely with the Nephrologist, Endocrinologist, primary care providers, and interdisciplinary care team
- Educate patients about medication adherence, nutrition, physical activity, and cardiovascular risk reduction
- Participate in quality improvement initiatives and continued development of the cardiometabolic program
- MD or DO degree
- Completion of an accredited Internal Medicine residency and Cardiovascular Disease fellowship
- Board Certified or Board Eligible in Cardiovascular Disease
- Active New York medical license or ability to obtain one
- DEA registration
- Current BLS certification
- ACLS certification
- Four to six years of clinical experience
- Experience in non-invasive cardiology, preventive cardiology, or cardiometabolic care
- Experience in community health, ambulatory care, or an FQHC setting
