Position Title: Limited Permit Doctor (LPD)

Location: Fairview Skilled Rehab & Nursing Center
Status: Full-Time / Part-Time
Position Summary
The Limited Permit Doctor provides supervised medical care to residents under the direct guidance of a fully licensed physician. The LPD supports the medical team in delivering high-quality, person-centered healthcare, ensuring compliance with New York State Department of Health regulations and all facility policies. This role is ideal for physicians awaiting full licensure who want hands-on experience in a skilled nursing and rehabilitation setting.
Key Responsibilities Clinical Care
Conduct resident assessments, physical exams, and follow-up evaluations under supervision.
Assist in diagnosing and treating acute and chronic medical conditions.
Develop and implement care plans in collaboration with the supervising physician and interdisciplinary team (IDT).
Monitor resident progress, update documentation, and address changes in condition promptly.
Participate in rounds, care conferences, and interdisciplinary meetings.
Documentation & Compliance
Complete all required clinical documentation in the electronic medical record (EMR) accurately and timely.
Ensure compliance with NYS DOH, CMS regulations, and facility policies.
Maintain proper supervision logs and licensure documentation as required for Limited Permit status.
Resident & Family Communication
Communicate updates, treatment plans, and changes in condition to residents and families in a professional, compassionate manner.
Educate residents and families about diagnoses, medications, and procedures, under supervision.
Collaboration & Professionalism
Work closely with nursing, rehab, dietary, social work, and administration to deliver holistic resident care.
Maintain high standards of ethical practice and professionalism.
Participate in quality improvement initiatives and performance audits.
Qualifications
Required:
Graduate of an accredited medical school.
Active Limited Permit issued by the New York State Education Department (NYSED).
Eligibility for full New York State medical licensure.
Strong clinical judgment and willingness to learn in a supervised setting.
Excellent communication and interpersonal skills.
Preferred:
Experience or interest in geriatrics, rehabilitation medicine, or long-term care.
Familiarity with EMR systems.
Bilingual skills (not required but helpful).
Physical Requirements
Ability to stand, walk, and move intermittently throughout the facility.
Ability to assist residents, review charts, and work in a fast-paced clinical environment.
