Our interdisciplinary team works closely together to support residents through recovery, rehabilitation, and long-term care, while maintaining strong communication with families and hospital partners.
Position OverviewThe Attending Physician is responsible for overseeing and managing the medical care of residents in the skilled rehabilitation and long-term care setting. This role includes clinical decision-making, collaboration with nursing and rehab teams, supervision of mid-level providers and limited permit physicians (if applicable), and ensuring regulatory and quality standards are met.
This position is ideal for physicians interested in geriatrics, post-acute care, internal medicine, or family medicine within a team-based environment.
Key ResponsibilitiesProvide comprehensive medical care to residents in skilled rehab and long-term care
Conduct initial evaluations, routine follow-ups, and acute change-in-condition assessments
Manage chronic and complex medical conditions
Oversee admissions, readmissions, and discharge planning
Review and sign orders, progress notes, and care plans
Collaborate with nursing leadership, rehabilitation therapists, social work, and administration
Participate in interdisciplinary care plan meetings
Communicate effectively with residents and families
Supervise and mentor limited permit physicians, nurse practitioners, or physician assistants (as applicable)
Ensure compliance with state, federal, and facility regulations
Support quality improvement initiatives and best clinical practices
Coordinate with hospitals, specialists, and outside providers as needed
MD or DO from an accredited medical school
Active New York State Medical License | Board Certified or Board Eligible in:
Internal Medicine
Family Medicine
Geriatrics (preferred, not required)
Strong clinical judgment and decision-making skills
Excellent communication and documentation abilities
Ability to work collaboratively within an interdisciplinary team
Comfort working in a skilled nursing and rehabilitation environment
Prior experience in:
Skilled nursing facilities
Post-acute or rehabilitation settings
Long-term care
Familiarity with EMR systems
Experience supervising mid-level providers or limited permit physicians
