Whatyou will accomplish in this role
- Performing history and physical examinations
- Documenting visits in electronic medical record in a timely manner
- Educating patients on their health conditions in a way they can understand
- Following up with patients more frequently for effective condition management
- Utilizing your support team to complete standing orders
- Ordering diagnostic tests based on assessments, developing treatment plans, prescribing drugs and other therapies as needed
- Making referrals as necessary, consulting with supervising medical provider when appropriate
- Maintaining patient confidentiality
- Participating in meetings
- Working collaboratively with other members of the medical team
- Collaborating with the management team on quality improvement initiatives
- Having an open mind to feedback, different perspectives, reflection, and growth
- Must have completed an accredited NP/PA program and be registered by the NYS Department of Education
- 3 years or more experience practicing as a Nurse Practitioner or MD/DO
- Experience working with a diverse and under-served patient population a plus
- Proven ability to work in a team environment
- eClinical
Works experience is a plus - Bilingual Spanish Speaker (Required)
- Highly knowledgeable and supportive staff with a diverse background including receptionists, Call Center Agents, Health Coaches, Medical Assistants, Practice Manager, Clinical Supervisor and HR
- Providers for Primary Care, GYN, Cardio, GI and Podiatry
