Weare seeking a Board-Certified Site Physician to join our team in Bronx, New York and provide comprehensive medical care to patients at a Community Health/Day Treatment Center. This position offers a consistent Monday–Friday schedule andthe opportunity to make a meaningful impact caring for a frail and elderly patient population. The Site Physician will oversee the professional, clinical,organizational, and administrative aspects of patient care and work closely with an interdisciplinary team to coordinate high-quality, patient-centered services.
Compensation& Benefits
- Base salary $240,000–$280,000 depending on experience, plus bonus
- Comprehensive benefits package
- Full-time Monday–Friday schedule, 8:00 AM–5:00 PM
- Combination of site-based and home-care services
- Collaborative interdisciplinary practice environment
- Graduate of an accredited medical school.
- Completion of Internal Medicine or Family Medicine residency.
- Board Certified in Internal Medicine or Family Medicine required.
- Geriatric Medicine certification, fellowship, or experience preferred.
- Minimum one year of experience caring for frail or elderly patients preferred.
- Home-care experience preferred; must be willing to provide home-care services.
- Current,unrestricted New York State medical license.
- ActiveDEA license.
- CurrentCAQH profile.
- Active Medicare and Medicaid provider IDs.
- Excellent clinical, communication, organizational, and interpersonal skills.
- No malpractice or loss-of-license history related to practice at the DTC.
- Bilingual skills a plus
