School Counselor (Transition Specialist)
HFM BOCES Special Education has an opening and is accepting applications for a 10-month, full-time, tenure track School Counselor (Transition Specialist). There may be an additional opportunity for summer work for the Extended School Year Program.
The Transition Specialist supports component school districts in meeting the post-secondary transition needs of students with disabilities, particularly seniors working toward graduation and the CDOS Commencement Credential. This position collaborates with Special Education and Guidance staff to coordinate transition planning, work-based learning opportunities, CDOS documentation, transcript review, and student progress monitoring.
Responsibilities include developing job site placements, tracking student hours, completing required CDOS paperwork, supervising job coaches, and determining appropriate student work experiences. The Transition Specialist also provides direct student support through career exploration, resume writing, classroom presentations, and individualized conversations that help students develop realistic post-secondary goals.
In addition, this role supports Special Education departments by reviewing IEPs for compliance, assisting teachers with transition-focused IEP development, and providing professional development related to Indicator 13 and state expectations. Knowledge of the NYS Work-Based Learning Manual, unregistered work-based learning programs, CDOS Credential requirements, and compliant IEP development is helpful.
