Nursing Home Social Worker
LeRoy Village Green RHCF is searching for a Social Worker to become a part of our facility’s compassionate care‑giving team. We are a 140‑bed skilled nursing facility located in the scenic Village of LeRoy located in Genesee County, New York.
Social Worker Requirements:- Master’s Degree in Social Work
- Previous Short Term/Sub Acute Experience
Duties include:
- Completing MDS's
- Maintain an open line of communication with residents and families
- Attending care plan meetings
- Discharge Planning
