Title: Medical Case Manager
Salary Range: $53,560.00
Position OverviewThe Senior Medical Case Manager (MCM) enhances traditional care teams and builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy. The Senior Medical Case Manager will be responsible for helping individuals navigate access to community services and other resources.
Responsibilities- Conducts intakes and verifies and documents program eligibility.
- Completes patient assessments and reassessments utilizing a strength-based model.
- Provides health education, risk reduction and adherence education to patients within the scope of discipline to assist patient in engagement and retention and reduce barriers to care to enhance health outcomes.
- Conducts outreach to re‑engage patients in care.
- Participates in ongoing training and regional collaboratives to enhance knowledge and skills.
- Participates in quality improvement trainings and activities to improve patient care.
- Demonstrates cultural competency in working with diverse populations.
- Performs job duties in compliance with systems and grant specific standards.
- Maintains data in AIRS and documents activities in the electronic health records as required.
- Develops, implements and updates care plans for patients.
- Facilitates linkage to resources for benefit and entitlement programs.
- Participates in case conferences.
- Support retention in care and reduce barriers by collaborating with medical, behavioral health, and social services providers.
- Develops linkages and refers patients for additional supportive services.
- Collaborate and provide guidance to the Peer as it pertains to programmatic requirements and client engagement.
- Assist and collaborate with preparation of monthly reports and site visits.
- Deliver health information using culturally appropriate terms and concepts.
- Link individuals to health care/social service resources.
- Provide informal counseling support and follow‑up.
- Support retention in care and reduce barriers by collaborating with medical, behavioral health, and social services providers.
- B.A. or B.S. with at least one (1) year of HIV or other chronic illness related field experience, preferred.
- Associate degree and three (3) years of such experience, or five (5) years of such experience.
- 2-3 years of case management related experience working in a community‑based setting.
- HIV/AIDS/STI knowledge including prevention, infection, and treatment.
- Knowledge of healthcare terminology.
- Basic computer skills for word processing and spreadsheet use.
- Knowledge of AIRS and eCW strongly preferred.
- Good communication skills.
- Ability to work in a culturally diverse environment.
- Lived experience preferred.
- Bilingual – Spanish a plus.
- Most work is completed in the community, with occasional work completed in office.
- Not substantially exposed to adverse environmental conditions (Typical of most clerical and administrative positions).
- Sedentary work equivalent to lifting approximately 10 pounds with occasional lifting and/or carrying of small objects.
- Frequent and extended walking and standing.
- Seldom bending, climbing, reaching and lifting.
- Follows clearly defined policies and procedures.
- Makes decisions within well‑defined parameters for the completion of own tasks only.
Provided with instructions on job requirements, methods, priorities and deadlines with particular attention to unusual or difficult aspects or tasks. There are regular or systematic reviews of progress, quality and output, usually at discrete times, but occasionally only upon completion.
