Overview
The Program Supervisor is responsible for providing program supervision and oversight to ensure the delivery of high-quality services and compliance with all regulations and guidelines for the Community Habilitation and/or Day Habilitation programs

- Hire, train and provide on‑going supervision to Direct Support Professionals (DSPs).
- Support program participants to identify and actualize outcomes and support needs.
- Monitor the quality of the service provided by DSPs through regular contacts, in‑person visits and other remote means of communication.
- Conduct periodic reviews of staff performance. Address areas in need of disciplinary action.
- Conduct periodic staff meetings.
- Provide DSPs with training and technical assistance.
- Follow up and rectify billing errors and data omissions.
- Participate in Life Plan meetings at least twice annually. Review the draft, finalized and/or amended Life Plan upon receipt to ensure accuracy and concurrency with Staff Action Plan.
- Review, write and update Staff Action Plan twice annually or as needed. Ensure that the Staff Action Plan addresses the individualized needs, wants and desires of the people supported.
- Document monthly summary of individuals’ progress (monthly notes) as well as conduct and document quarterly review of service.
- Maintain the records of persons supported in accordance with OPWDD Regulations (i.e., LCED, Life Plan, enrollment documents, evaluations etc.).
- Report all significant and reportable incidents and/or suspected instances of abuse and neglect immediately and by following established incident procedures.
- Attend internal and external meetings and trainings as scheduled or assigned by the Assistant Director.
- Review and approve biweekly payroll on Therap as well as current HRIS system.
- Keep the Assistant Vice President informed of important/relevant information related to services and safety of the individuals.
- Other duties as assigned.
- Other: To hold a cellular 24 hours, seven days per week.

Education and Experience Requirements:
- Minimum of Associate’s degree or Bachelor’s degree in Health and Human Services or related field is preferred.
- Minimum of two years‑experience in the field of Developmental Disabilities with, at least, one‑year experience in program supervision is preferred.
Physical Requirements:
Prolonged periods of sitting at a desk and working on a computer. While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to sit; stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 15 pounds.
