• Minimum of Bachelor of Science Degree in Physical Therapy from accredited institute of learning
• Licensed and registered as a PT in New York State
• Minimum of 1 year experience in a health care setting;
  Home Care experience highly preferred
• Must be able to provide own transportation in order to make visits in assigned area(s); vehicle/drivers license may be required
