Evening Shift - 7.5 Hours (United States of America)

The Social Worker will provide clinical and case management care to patients on a mobile crisis team setting. Documentation must be in a timely and quality manner. Conduct individual, group, and family therapy. Experience treating concurring substance abuse and mental illness required for position.

• Master's degree required.
• NYS Registration/Certification as a Social Worker required.
• Prior experience in a hospital or community health agency is preferred.
• Experience with assigned patient population and utilization of PM Schedule and LMSW/LCSW required.
• Experience treating concurring substance abuse and mental illness required.

Evenings with every weekend and holidays; 3p-11p

Salary Range: $41.694/hr - $50.88/hr (Commensurate with Experience)
