Concepts of Health Care, Inc is currently looking for a part-time RN to become a team player for our licensed home care agency. Typical office hours would be Monday – Wednesday 8 am – 2 pm in Ballston Spa. The ideal candidate should be polite, personable, professional and punctual.
Job responsibilities- Being responsible for visiting/assessing clients in their homes.
- Communicating and coordinating with doctors, clients, families and staff.
- Completing nursing assessments & medical documentation.
- Aide supervision, training and scheduling.
- Adhering to DOH regulations as well as agency Policies and Procedures.
- Overseeing the agencies DOH Personal Care Aide Training Program.
- On-call.
Salary: $630.00/week + mileage.
