- Licensed Clinical Social Worker in New York state
- Primary care or hospital setting experience preferred
- In-person position with no remote option
- Provides behavioral health consultation across all age groups
- Multiple treatment modalities: CBT, DBT, MI, trauma-focused
- Behavioral health care coordination and transition management
- Crisis intervention and complex family dynamics support
