We are looking for an indivual who can take on the roll as a dir of social work at our SNF. This job includes but is not limited to:

• setting up care plan meetings
• working with other departments to coordinate best plan of action for each resident
• strong follow up, and people skills
• evaluating residents using the MDS assessment
• working well with assisted living facilities as well as home care agencies to ensure a safe and timely discharge for each individual resident

• Lead and manage social work services within the skilled nursing facility (SNF).
• Coordinate care plan meetings and collaborate with interdisciplinary teams.
• Conduct follow-up and maintain strong communication with residents and families.
• Evaluate residents using the MDS assessment.
• Collaborate with assisted living facilities and home care agencies to facilitate safe and timely discharges.

• Experience in social work leadership or management within a skilled nursing facility or similar setting (preferred).
• Ability to coordinate across departments and with external partners.
• Strong follow-up skills and interpersonal communication abilities.
• Knowledge of MDS assessments and resident evaluation processes.
• Experience working with assisted living facilities and home care agencies is a plus.
