HCBS Supervisor – Home and Community-Based Services (Full-Time)

Job Type: Full-Time;
Location: Brooklyn, NY;
Salary: $90,000 – $100,000 (Competitive, based on experience)

The HCBS Supervisor oversees daily HCBS program operations, supervises case managers and direct support staff, and ensures compliance with Medicaid, state, and HCBS regulations. This leadership role focuses on staff supervision, care coordination, documentation, quality assurance, and client advocacy.

• Supervise HCBS staff, including case managers and direct support professionals.
• Assign and monitor caseloads for timely service delivery.
• Ensure compliance with Medicaid and HCBS requirements.
• Oversee individualized service plans and person-centered care.
• Conduct staff training, supervision, and performance evaluations.
• Review documentation in EHR and case management systems.
• Serve as a liaison for clients, families, and community partners.
• Participate in audits, quality improvement, and crisis response.

• Bachelor’s degree in Social Work, Psychology, Nursing, Human Services, or a related field (Master’s preferred).
• 2–3+ years of experience in HCBS, Medicaid, or community-based services.
• Supervisory experience preferred.
• Strong knowledge of Medicaid compliance and documentation.
• Excellent leadership, communication, and organizational skills.
• Proficiency with EHR or care coordination systems.
