Overview
Ronald McDonald House New York (RMH-NY) is partnering with Ameri Corps to expand access to care and improve health outcomes for medically complex children and their families. RMH-NY is seeking Ameri Corps members to serve as Patient Navigators as part of New York State’s Medicaid Waiver initiative. In this role, Patient Navigators will perform telephone outreach to families eligible for Medicaid.
Responsibilities- Conduct screenings for Health-Related Social Needs (HRSNs)
- Facilitate referrals and connect families to essential services
- Provide care coordination in collaboration with hospital partners, Social Care Networks (SCNs), and community-based organizations
These efforts will strengthen RMH-NY’s capacity to help New York families achieve better health by supporting them across medical and social systems of care.
