Division/Program Summary

The Division of Disease Control’s mission is to safeguard the health of New Yorkers through identification, surveillance, treatment, control, and prevention of infectious diseases, which is achieved through varied and interrelated endeavors of its seven Bureaus. The Bureau of Public Health Clinics (BPHC) mission is to promote a healthy community by providing New Yorkers with the resources needed to make informed and empowered health decisions;

identify and treat tuberculosis and provide immunization and sexual health services regardless of ability to pay or immigration status. BPHC provides direct clinic services to people seeking health care; monitors disease trends; provides education and training to service providers and community groups; conducts research; and develops policies and programs to deliver high quality clinical care which best serve New Yorkers.

Job Duties and Responsibilities
  • Interview / re-interview and educate patients infected with a communicable disease.
  • Elicit contacts and other information in order to locate or identify the source of infection and prevent further spread of the disease.
  • Conduct surveillance activities including administering diagnostic tests for TB and collecting/transporting specimens.
  • Conduct field investigations on communicable disease patients to locate and return lost patients to medical follow‑up and to assess the home environment.
  • Refer patients to the appropriate diagnostic facility for medical examination and treatment.
  • Observe patients with a communicable disease ingest prescribed medication.
  • Establish and maintain communication with co‑workers, supervisors, managers, and community partners.
  • Manage and monitor patients suspected, confirmed to have disease or are contacts of infectious patients from diagnosis through completion of treatment.
  • Conduct HIV counseling, testing & post‑test follow‑up.
  • Conduct QA activities i.e., pouch reviews and field audits for PHA level I(s) and Assistant PHA(s).
  • Assume the duties of supervisor in his/her absence to ensure continuity of case management.
