Overview

Hillcrest Health & Rehab is seeking an experienced and compassionate Director of Social Services to lead our patient transitions and social services functions. This is a key leadership role responsible for ensuring smooth care transitions, strong census management, regulatory compliance, and exceptional support for patients and families. Responsibilities- Leading and managing the Transitions department and team members
- Overseeing census development, reconciliation, and maintenance to meet or exceed budgeted goals
- Managing referrals and coordinating with Central Transitions, referral sources, and leadership to maximize appropriate admissions
- Directing discharge planning and coordination of care across Hillcrest service lines and affiliated entities
- Ensuring compliance with all state, federal, and regulatory requirements related to transitions and documentation
- Overseeing EMR processes related to admissions, discharges, insurance, Medicaid screenings, and
- Monitoring insurance updates, copayments, and financial communications to support fiscal responsibility
- Serving as a key point of communication for patient and family concerns, partnering with the Administrator to resolve issues
- Acting as second in command and assuming Administrator duties as needed
- Participating in QAPI and leading assigned committees and departmental meetings
- Supporting outreach and relationship-building with hospitals, discharge planners, physicians, and referral partners
- Bachelor’s or Master’s degree in Social Work from a CSWE-accredited program or related healthcare or health services degree
- Minimum of 2 years of experience in social work, case management, or a related field
- Minimum of 2 years of experience in discharge planning, social services, or case management in a post-acute or acute care setting
- Strong leadership, communication, and organizational skills
- Ability to remain calm, flexible, and professional in emotionally challenging situations
- Detail-oriented with the ability to manage multiple priorities in a fast-paced environment
- Proficient with technology and electronic medical records
- Commitment to collaboration, accountability, and service excellence
- Ability to lift up to 35 pounds
- Ability to sit, stand, bend, and move throughout the workday
- Must be able to read, write, and speak English
