Position Overview
The Case Manager works with children and adolescents in community residence, supervising, caring for, and building skills in line with each youth’s individual service and treatment plans. They serve as a positive role model, providing therapeutic support, trauma‑informed care, and supervision, all in accordance with the standards of regulatory bodies and the Jewish Board of Family & Children’s Services.
Key Essential Functions- Interview and assess youth and families for the matching process and conduct intake assessments and documentation.
- Orient families to services, collaborate with schools, outpatient mental health providers, and other community providers.
- Develop culturally affirming, trauma‑informed, individualized service plans in collaboration with youth, families, and providers.
- Identify client strengths and behavioral health needs; recommend services and interventions.
- Provide clinical coordination of services and after‑care as required.
- Deliver crisis intervention and provide feedback to the supervisor and treatment team.
- Work as part of an interdisciplinary therapeutic team; participate in meetings, trainings, and supervision.
- Maintain documentation per Jewish Board and regulatory agency standards.
- Maintain professional behaviors and ethical standards per licensing board and Jewish Board policies.
- Travel and navigate the community to provide services where most beneficial.
- Perform day‑to‑day care duties, including hygiene, social guidance, leisure planning, and food preparation.
- Develop, supervise, and implement therapeutic recreational activities.
- Assist in maintaining a safe and clean environment.
- Perform services in community residence, families’ homes, community settings, and via telehealth (when applicable).
- Additional duties as assigned.
- Work evenings and weekends as needed.
- Attend and complete Therapeutic Crisis Intervention (TCI) certification and recertifications.
- Participate in ongoing training and consultation of the evidence‑based model utilized within the program.
- Provide supervision of youth and oversight of the program.
- Bachelor’s Degree required.
- Valid driver’s license required.
- Excellent engagement skills.
- Strong verbal and written communication skills.
- Attention to detail.
- Ability to work independently and as part of a team.
- Ability to utilize an electronic health record and Microsoft Office.
- Bilingual Spanish/English preferred.
- Verbal and communication skills.
- Team player.
