Position OverviewPOSITION OVERVIEW:
Under the supervision of the Clinical Supervisor, the Housing/Case Manager will provide case management services to a caseload and provide housing assistance within the facility.
KEY
ESSENTIAL FUNCTIONS:
Case Management:
- Provide case management services to a caseload of up to 15
- Manage and develop systems in the facility.
- Complete all paperwork for charting as required by funding source
- Conduct groups periodically or as directed by management
Housing Services:
- Provide Housing Assistance to clients
- Complete Housing Assessments and Referrals
- Complete HRA 2010-E’s (CAPS)
ADDITIONAL FUNCTIONS MAY INCLUDE:
- Attend management trainings, team meetings, and staff meetings.
- Provide assistance within the facility where needed.
- Performs other duties as required by the Clinical Supervisor.
EDUCATIONAL / TRAINING
REQUIRED:
- Either CASAC or Bachelor degree Preferred
- GED/High School Diploma or Associates Degree Required
- CASAC-T required
EXPERIENCE REQUIRED / LANGUAGE PREFERENCE:
- Minimum 2 years’ experience with population in a residential setting or related field
COMPUTER
SKILLS REQUIRED:
- Proficient in Microsoft Office and related programs
