Under the direction of the Program Manager of the H.I.P.P program, the Patient Navigator is responsible for conducting outreach and in‑reach activities, assist with intakes and provide education to potential PEP/PrEP and HIV positive newly diagnosed patients seeking services. The Patient Navigator also assists in the coordination of medical appointments, troubleshoots patient insurance issues; enters data into AIRS, conducts follow up with clients, and provides retention and adherence counseling to PEP/PrEP and primary care patients.
DutiesAnd Responsibilities
- Assesses needs of patients seeking and accessing PEP and PrEP related services.
- Assist providers with identifying patients for PEP/PrEP and assist with intakes/program enrollment, service navigation, education and follow up around PEP and PrEP services.
- Assists with data collection and data entry for the AIDS Institute Reporting System and CHN’s Electronic Medical Record.
- Coordinates medical services with clinic staff.
- Provide a wide range of community education and increase awareness about Treatment as Prevention for HIV positive individuals and Pre‑Exposure Prophylaxis (PrEP) Post‑Exposure Prophylaxis (PEP) for those at high risk for HIV infection.
- Participates in health fairs, tabling and joint health activities with other community agencies.
- Participate in trainings and support services.
- Maintains strict confidentiality of all patient related information.
- Facilitates PEP/PrEP related services for health center patients as appropriate with respect to their confidentiality and privacy.
- Increase the number of undiagnosed persons who know their HIV status and are linked to care.
- Identify, link, and retain persons diagnosed with HIV and ensure prompt access to medical care, partner, and prevention services.
- Navigate patients into integrated STD and HVC education and linkage to STD and HVC screening and treatment.
- High School diploma or G.E.D. preferred with two years’ experience in community work and patient navigation/linkage.
