Description Of Duties And Responsibilities

Conducts direct street outreach to individuals in need of addictions medication services.

Establishes a respectful, trusting relationship with program participants.

Assists participants to identify and build on their strengths and resiliencies.

Assists participants in identifying basic needs.

Provides referrals for medication assisted treatment of substance use disorders.

Provides guidance and resources to help participants access healthcare, behavioral healthcare, public assistance, housing, nutritional assistance and health insurance.

Provide training to program participants and distributes medication for opioid overdose reversal.

Distributes harm reduction supplies, including: wound care kits, safer sex kits, sharps disposal and medication

disposal kits, substance supply test kits for fentanyl and other synthetic drugs.

Escorts participants to clinic for assessment and same day medication initiation.

Assists patients with medication access.

Facilitates warm hand-offs with network service providers.

Acts as advocate on behalf of program participants with professional staff and network service providers.

Participates in team meetings and service planning conferences.

Documents delivery of services in case record.

Participates in in-service training according to program requirements.

Qualifications For Position

New York State Certified Recovery Peer Advocate or Provisional CRPA preferred.

CARC preferred.

Lived experience in recovery from substance use disorder.

2 years of direct care work experience with individuals at risk for, or with a substance use disorder.

Skills And Knowledge Required

Demonstrates effective communication skills; recognizes and uses person-centered language.

Recognizes that there are multiple pathways to recovery/wellness.

Practices a strengths-based approach to recovery/wellness.

Proficient with Microsoft Office applications

Bi-lingual English/Spanish preferred

Ability to engage active substance users and their families

Amount of travel and any other special conditions or requirements

Frequent local travel required
