Nurse Care Manager, HART
Program Overview
:
The Nurse Care Manager, Psychiatric Services reports to the Medical Director of Psychiatric Services and the Chief Medical Officer. Collaborates with interdisciplinary team including Psychiatry, Primary Care, Drug User Health, Nursing, PEQA/QA, Healthcare Operations, Supportive Housing program staff, Transitional Housing Program staff and Permanent housing program staff.
The essential duties of the Nurse Care Manager, HART include but are not limited to the following activities
Clinical
- Meets with new clients to review and document psychiatric and medical history and assess learning needs and social determinants of health metrics as they relate to psychiatry, medical, and substance use
- Work in conjunction with HART OT to develop plans for HART clients.
- Provides counseling and education for clients about their care plan and healthcare options
- Provides case management support for HART clients, including triage of and assistance with psychiatric, primary care or pharmacy issues that arise in between appointments.
- Collaborates with psychiatry, nursing, primary care, drug user health programs, housing programs and external providers
- Provides care coordination with EMS, hospitals, and higher levels of care
- Facilitates referrals to community-based specialty providers, and other community-base resources to address social determinants of health.
- Documents patient interactions in the Electronic Health Record
- Participate in weekly supervision
- Administer LAIs as needed
Quality Assurance
- Assists in development of quality assurance measures
- Assists and supports Medical Director, CQI Project Manager with chart reviews and clinical data review.
Note:
This role will travel as needed to support HART clients at all PRI housing locations.
Qualifications:
Education Credentials/Requirements:
New York State Registered Nurse & BLS Certification.
Experience:
2-3 years of experience working registered nurse
Preferred Qualifications:
Working in community settings with people experiencing serious
mental illness and people experiencing homelessness.
Communication:
Must have excellent oral and written skills.
Organizational
Skills:
Must have the ability to work independently, prioritize tasks, and
create and coordinate schedules.
Interpersonal
Skills:
Must have the ability to interface with clients and staff with a broad range of socioeconomic and cultural backgrounds.
Computer
Skills:
Must have knowledge of Microsoft Office, especially Word and Excel. Knowledge of eClinical
Works is a plus
