The responsibilities of this position are to provide collaborative drug therapy management utilizing collaborative practice agreements detailing prescriptive privileges in both the inpatient and outpatient settings, allowing for medication order initiation, modification, and discontinuation as well as laboratory ordering for medication monitoring purposes. Patient care responsibilities of this position are to round daily with an interdisciplinary team and provide drug information, comprehensive pharmacotherapy consultations, pharmacokinetic dosing, medication reconciliation, and drug discharge counseling and to participate in codes and formulary management.
Opportunities for direct patient care within the ambulatory care clinics are also available. The Clinical Pharmacist Specialist will also administer lectures to the medical/nursing staff, participate in key hospital-wide committees, and serve as a liaison between the pharmacy and medical/nursing staff.
Another responsibility of the position is to serve as preceptor for PGY1 and PGY2 pharmacy residency programs. The role encompasses serving as a faculty member, role model, mentor and preceptor, through contributions to the residency learning experience, recruitment, and accreditation requirements.
- Participate in daily rounds with med‑surg teams
- Collaborate with physicians, medical residents and other prescribers to ensure safe and cost‑effective medication use including therapeutic substitution
- Provide drug information to the medical, nursing, and pharmacy staffs, and other allied healthcare professionals
- Monitor for medication efficacy and toxicity
- Manage sub‑therapeutic and supra‑therapeutic drug levels and provide therapeutic drug level monitoring
- Administer medication reconciliation and patient counseling
- Detect, report, and manage adverse drug events
- Detect, report, and manage medication errors
- Participate in collaborative drug therapy management including ordering laboratory tests, ordering medication, and documenting notes in medical charts
- Offer opportunities for direct patient care within the ambulatory care clinics
- Conduct medication‑use evaluations and analyze data generated regarding prescribing trends and proper medication usage
- Serve as chair, director, team leader in committees, special projects or clinical programs
- Participate in hospital‑wide committees such as the Pharmacy and Therapeutics Committee
- Document pharmacotherapy interventions and provide pharmacoeconomic reports
- Serve as a preceptor for PGY1 and PGY2 pharmacy residents
- Oversight of resident's progression, learning experiences, scheduling, and evaluations
- Mentor residents through clinical practice, research and presentation and publication scholarship
- Oversee daily activities of pharmacy practice and specialty residency programs
- Participate in pharmacy student educational activities
- Perform all other duties as required
PGY1 pharmacy residency required. Pharm
D required. 2 years of experience. PGY2 preferred.
