The Role

The Outreach Worker provides community-based outreach, engagement, and recovery support services to individuals impacted by substance use disorders and mental health challenges. Working under the supervision of the Program Coordinator, the Outreach Worker engages individuals in community settings, builds trusting relationships, and supports access to treatment, recovery services, and basic needs using a trauma-informed, harm-reduction, and culturally responsive approach.

Responsibilities

What You Will Do

Community Outreach & Engagement

  • Conduct outreach in community settings, including shelters, hospitals, treatment programs, street-based locations, and other community sites.
  • Build rapport and engage individuals who may be disconnected from care or reluctant to access services.
  • Provide education on available recovery, treatment, and support services.
  • Support engagement of hard-to-reach or high-risk populations.

Member Support & Navigation

  • Assist individuals with referrals and linkage to substance use treatment, mental health services, medical care, housing, and social supports.
  • Support appointment scheduling, accompaniment, and follow-up as needed.
  • Promote self-advocacy, recovery goals, and continuity of care.
  • Respond to member needs, concerns, and emerging crises in collaboration with the Program Coordinator.
Qualifications

Who You Will Be

  • High School Diploma or Equivalent and hold a CRPA or CASAC-T certification.
  • Minimum of 1–3 years of experience in substance use, mental health, peer support, or community-based services.
  • Demonstrated knowledge of or lived experience with OUD and/or StUD.
  • Strong understanding of recovery-oriented systems of care.
  • Experience working with individuals from diverse racial, ethnic, and socioeconomic backgrounds.
  • Ability to engage individuals with complex needs using a nonjudgmental, strengths-based approach.
  • Strong interpersonal, communication, and documentation skills.
