Summit Home Health Care – Home Care Community Liaison / Marketing Specialist

Salary: $52,000 per Year, plus a very generous productivity Bonus Structure.

• Bring workable patient referrals to the agency
• Develop new referral sources in the community
• Strengthen relationships with new and existing referral sources – daily visits to organizations, physician offices, hospitals, assisted living facilities and adult day care to educate and inform referral sources and patients of home health options
• Pre‑plan weekly marketing activities and follow‑through
• Event coordination and community liaison – participate in health fairs, awareness days, etc.

• Prior experience in Home Health Care is recommended, but we will train the right individual
• Knowledge and understanding of Medicaid/Medicare guidelines and MLTCs is a plus
• Excellent customer service, attention to detail and communication skills a must
