Location: New York

Join my Laurel as a Clinical Liaison in Brooklyn, NY. We are seeking a mission-driven Registered Nurse (RN), Social Worker (LMSW/LCSW), or Paramedic to act as the primary bridge between hospital leadership and our innovative acute care‑at‑home program. In this high‑impact role, you will drive Acute Care Management by identifying eligible patients and fostering relationships with Interdisciplinary Teams.

Compensation & Perks
  • Salary: $100,000 – $105,000 base salary + performance bonus.

Core Responsibilities
  • Stakeholder Engagement:
    Cultivate strong professional relationships with hospital leadership, case managers, and discharge planners to integrate my Laurel into care pathways.
  • Referral Growth:
    Proactively identify and develop new referral sources within the Brooklyn hospital system to expand our service line.
  • Utilization Review:
    Conduct high‑acuity Clinical Chart Reviews to screen patients for eligibility and facilitate the enrollment process.
  • Patient Advocacy:
    Educate patients and families on the benefits of acute care‑at‑home and provide seamless Discharge Planning support.
  • Operational Integration:
    Collaborate with hospital staff and the my Laurel clinical team to ensure accurate EHR Documentation and patient handoffs.
Qualifications & Requirements
  • Active Licensure:
    Must hold a current, unrestricted New York State license (RN, LMSW/LCSW, or Paramedic).
  • Clinical Background: 4+ years of clinical experience in an Inpatient or Emergency Room (ER) setting.
  • Technical Savvy:
    Proficiency in Electronic Health Records (EHR) and Google Workspace (Docs, Sheets, Slides).
  • Adaptability:
    Ability to work flexible 8‑hour shifts (e.g., 9:00 am – 5:30 pm or 11:00 am – 7:30 pm) to meet business needs.
  • Connector Mindset:
    Excellent communication skills with a knack for business development and relationship management.
