Social Worker – Hospice – Full-Time Days

You will serve as Hospice social worker for patients and their families, providing support, therapeutic interventions, education, and patient advocacy tailored to the needs of the specific client population. We're seeking an experienced Hospice Social worker dedicated to providing compassionate and empathetic care to our patients.

This is a full-time position located at Jansen Hospice and Palliative Care, a community-based Certified Hospice Agency. This position will support development of Jansen Compassionate extubation program on site s position requires traveling to multiple locations providing care in patients' homes, and other facilities. Assignments may include working on holidays and / or weekends as needed.

Preferred Criteria

· State and local community services

Required Criteria

· Master's Degree in Social Work (MSW)

· Licensed Masters Social Work (LMSW) by the state of NY within 1 year of hire

· Knowledge of Psychosocial Assessment

· Communication Skills
