• Coordinate care for individuals in the Health Homes program
• Develop and maintain person-centered care plans
• Build steady relationships with members and providers
• Connect individuals to medical, behavioral health, and social supports
• Complete timely, accurate documentation
• Work independently with strong team and leadership support

• Have a bachelor’s degree in human services or related experience
• Are organized, dependable, and communicative
• Prefer clear expectations and manageable workloads
• Value professionalism and person-centered care
