Job Title: Hospital Liaison
Department: Admissions / Business Development
Reports To: Regional Director of Marketing / Administrator
Location: Assigned Hospitals & Skilled Nursing Facilities
Position Summary:
The Hospital Liaison serves as the primary point of contact between hospitals and Citadel Care Centers. This individual is responsible for building strong relationships with hospital case managers, discharge planners, physicians, and social workers to facilitate seamless patient transitions into Citadel’s post-acute care facilities. The role involves marketing the facility’s services, coordinating admissions, and maintaining census goals through effective outreach and communication.
Key Responsibilities:
- Establish and maintain strong relationships with hospital discharge teams, physicians, and community partners.
- Assess potential admissions for clinical appropriateness in collaboration with facility nursing and admissions teams.
- Coordinate and expedite the admissions process from hospital to facility, including gathering clinical data, insurance authorizations, and transportation arrangements.
- Promote Citadel Care Centers’ services and programs to hospitals and referral sources.
- Track referral activity, outcomes, and trends; maintain an up-to-date CRM or referral database.
- Represent the facility at hospital-based events, case management meetings, and community outreach opportunities.
- Identify new business opportunities and provide regular market feedback to leadership.
- Meet or exceed monthly census and referral goals.
Qualifications:
- Bachelor’s Degree in Healthcare, Marketing, Nursing, or related field preferred.
- Minimum 2 years of experience in healthcare admissions, marketing, or case management (preferably in post-acute/skilled nursing).
- Knowledge of Medicare, Medicaid, and managed care guidelines.
- Excellent interpersonal, communication, and organizational skills.
- Ability to work independently and manage time across multiple sites.
- Clinical background (RN, LPN, Social Work) is a plus but not required.
Work Environment:
- Frequent travel between assigned hospitals and care centers.
- Some after-hours or weekend work may be required depending on patient needs.
