TitleIntegrated Care Coordinator, Enrollment
Position SummaryThe Enrollment Integrated Care Coordinator serves as a critical link between patients and the services, ensuring seamless care coordination, particularly for underserved and high‑needs populations. This role removes barriers to care by connecting patients with primary care, specialty services, behavioral health, and community resources to support comprehensive, patient‑centered care. As an Enrollment‑focused ICC, this position is also the point of contact for patients referred by internal and external partner organizations into Intention Healthcare, a subsidiary of Essen serving homebound and complex patients.
Acting as the initial voice of Intention, the ICC clearly communicates the value and benefits of our Care Management services, supports patients through the onboarding process, and ensures a smooth, supportive entry into care.
- Patient Engagement:
Building trust and rapport as the first voice patients hear during their introduction to Intention Healthcare. - Program
Education:
Serve as an internal expert on the member and caregiver onboarding experience, owning program outreach, education, and remote onboarding from start to finish. - Enrollment Support:
Manage a call list to effectively educate target members and their care teams on the benefits of the assigned program for outreach. - Growth Enablement:
Driving participation and supporting organizational enrollment goals through consistent outreach and follow‑ups, maintaining high patient engagement and consent rates. - Patient Satisfaction:
Ensuring that patients are provided efficient care coordination for continuity and valued services. - Demonstrate call‑handling skills: tone, pacing, clarity/articulation, professionalism, empathy, active listening, and ability to engage patients in conversation while maintaining/implementing Intention Healthcare’s quality assurance expectations.
- Collaborate with the Intention Healthcare Clinical Team in scheduling of in‑person or virtual provider visits to onboard new patients and their caregivers to the program.
- Educate patients and families about health conditions, treatment options, and self‑management strategies.
- Advocate for patients within the healthcare system to ensure access to appropriate service.
- Distill and share learnings from outreach and onboarding conversations with Client Success, Business Operations, and Product teams to facilitate continuous improvement on both program positioning and mobile/tablet application adoption.
- Bachelor's degree (Preferred)
- Relevant certifications in healthcare, social services, or related fields considered.
- 1-2 years of direct patient care experience; inside sales experience in healthcare preferred.
- Experience in Sales, Care Coordination, or Case Management.
- Comfortable being on the phone throughout the day.
- Familiarity with Electronic Health Records (EHR).
- Previous experience in outreach, community engagement, social services, or related field.
- Fluent bilingual Spanish and English (Preferred)
- Excellent communication and public speaking skills.
- Ability to work independently and travel within the Bronx.
- Strong organizational and time‑management skills.
- Cultural sensitivity and ability to work with diverse, underserved populations.
- Proficiency in motivational interviewing and health coaching techniques.
- Proficiency in Microsoft suite including Teams and Excel.
- Pay $22.00 - $25.00 per hour
