Title

Integrated Care Coordinator, Enrollment

Position Summary

The Enrollment Integrated Care Coordinator serves as a critical link between patients and the services, ensuring seamless care coordination, particularly for underserved and high‑needs populations. This role removes barriers to care by connecting patients with primary care, specialty services, behavioral health, and community resources to support comprehensive, patient‑centered care. As an Enrollment‑focused ICC, this position is also the point of contact for patients referred by internal and external partner organizations into Intention Healthcare, a subsidiary of Essen serving homebound and complex patients.

Acting as the initial voice of Intention, the ICC clearly communicates the value and benefits of our Care Management services, supports patients through the onboarding process, and ensures a smooth, supportive entry into care.
