Key Responsibilities Serve as the primary contact for members and their support team, providing comprehensive case management Assess member needs and develop a comprehensive Care Management Plan with a multidisciplinary team Connect members to social services, advocate for their needs, and monitor treatment adherence
Required Qualifications , Training, and Education Fluency in Spanish is required High School Diploma or GED required;
Bachelor's degree in a related field preferred 2+ years of experience in care management or mental health services with Medi-Cal members Current California driver's license with an acceptable driving record Comfortable working with diverse populations in various environments
