Position: Licensed Clinical Social Worker - OASIS

Location: New York, NY

Position Type: Regular Full-Time

We are seeking a Licensed Clinical Social Worker - OASIS to join our team. You will provide emotional and administrative support to clients in either individual or group settings. Guided by a Population Health model of care. We have five integrated clinical divisions offering urgent care, primary care, and specialty services, as well as nursing home staffing and care management.

• Assess the nature of clients' situations by interviewing and reviewing personal history
• Develop and execute individual treatment plans
• Determine appropriate milestones to gauge client progress
• Communicate client updates to all relevant parties
• Facilitate referrals to other healthcare professionals and programs
• Maintain accurate client documentation
• Conduct crisis assessments/triage with appropriate interventions
• Provision of other services related to wellness
• Other related activities designed to provide integrated health care services

• Master’s Degree required
• LCSW (New York license required)
• One to two years’ experience providing clinical services in the mental health field
• Bilingual English/Spanish a plus
• Hybrid work model
• Ability to provide specialized mental health care, often focusing on trauma, addiction, and specialized therapies like EMDR or DBT
