  • Valid license as an LMSW issued by the NYSED;
  • Valid license as an LCSW issued by the NYSED.
Duties & Responsibilities

POSITION OVERVIEW

The Social Worker II reports directly to the Director of Social Work. This position is expected to fulfill the duties and responsibilities assigned in a consistent and affective manner to meet standards of performance established by New York City Health+ Hospitals and Coler Facility Missions, Vision, and Values. This position is responsible for the provision of the scope of social work services to the residents and their significant others at Coler.

ESSENTIAL

FUNCTIONS
  • Maintains a caseload of several units, completing all necessary documentation, providing screening, assessment, psychosocial support, and assisting with concerns, complaints and grievances.
  • Provides discharge planning guidance and assistance, ensuring timely safe discharges of discharge-ready residents.
  • Confers and consults with professionals in other disciplines to determine and implement a multidisciplinary approach to resident care and to develop care plans, evaluate residents' progress, develop discharge and aftercare plans and recommend alternative action or treatment.
  • Participates in unit team meetings and multidisciplinary meetings to discuss resident care and discharge planning.
  • Provides outreach services, makes referrals to other governmental, public and/or community agencies, prepares residents to accept services, provides direct social work services on high-risk cases and may escort residents in connection with community activities and referrals, as needed.
  • Participates in departmental coverage when needed.
  • Orients and educates members of other professional disciplines on social work concepts and functions.
  • Assists with voter registration and voting process.
  • Conducts performance improvement activities within the department related to social work practice.
  • May provide supervision for social work students, if permitted by the Director of Social Work.
  • Performs other duties as assigned by the Director of Social Work.
Prerequisites

Experience in Post-Acute Care setting.
