Enrollment Coordinator

Position Summary

The Enrollment Coordinator serves as a critical link between patients and the services, ensuring seamless care coordination, particularly for underserved and high‑needs populations. This role removes barriers to care by connecting patients with primary care, specialty services, behavioral health, and community resources to support comprehensive, patient‑centered care. As an Enrollment‑focused ICC, this position is also the point of contact for patients referred by internal and external partner organizations into Intention Healthcare, a subsidiary of Essen serving homebound and complex patients.

This position is ideal for individuals looking to make a meaningful impact in the healthcare field while working in a supportive team environment. You'll be part of an organization that has demonstrated over 25 years of commitment to innovating healthcare delivery for underserved communities, with opportunities to grow alongside our rapidly expanding multispecialty medical group.
