Position:
Occupational Therapist (OT)

Duties and Responsibilities:
  • Evaluate referred residents to determine their functional level and potential for rehabilitation.

  • Provide direct care and develop individualized treatment plans to help residents regain independence in activities of daily living (ADLs).

  • Establish treatment goals based on each resident’s abilities, limitations, and physician recommendations.

  • Implement therapeutic interventions to improve strength, coordination, mobility, cognition, and self-care skills.

  • Educate residents and their families on home programs and adaptive techniques.

  • Complete documentation including evaluations, weekly progress notes, recertifications, and discharge summaries.

  • Collaborate with the interdisciplinary team to ensure a resident-centered approach to care.

  • Adhere to all applicable federal and state regulations, including HIPAA and Residents' Rights.

  • Perform additional duties as assigned by the Director of Rehabilitation.

Requirements/Qualifications:
  • Graduate of an accredited Occupational Therapy program.

  • Valid Occupational Therapy license and registration for the state of practice.

  • Experience in long-term care or geriatrics is preferred but not required.

  • Strong communication and organizational skills.

  • Compassionate, team-oriented, and committed to resident-centered care.
