Medical Specialist 2

The Medical Specialist 2 provides primary medical care, working in collaboration with other physician(s) and nurse practitioners or midlevel providers for adults with developmental disabilities and co-occurring conditions (ie: mental health). Services are provided primarily through the Adirondack Clinic located in Corinth, with satellite offices in Schenectady at 500 Balltown Rd, and Schuylerville Day Hab at 99 Broad Street in Schuylerville.

Compressed schedule of 4 ten hour days may be discussed ies would include but not be limited to review and establish medical histories, perform physical examinations, and manage the medical care of individuals with developmental disabilities. Maintain timely documentation in an electronic health record and provide limited telehealth per individual's request/need. Work collaboratively with a multidisciplinary team to integrate care with specialty providers and other clinicians.

Participate in "On-Call" telephone coverage for off hours with team (Currently 1 in
5). Cover CATH residents as necessary.

Additionally:

  • Work collaboratively with Nursing, Psychiatry, physical/occupational/speech therapists and nutrition services to develop individualized therapy plans as necessary.
  • Review discharge plans from hospitalizations to resume care in residences. Educate staff on hospital discharge plans if necessary.
  • Prescribe medications and other treatments to treat medical issues.
  • Maintain medical records in the EHR system and provide justification for billed services. Including signing EHR treatment plans, requests, and routine reviews from services listed above.
  • Discuss patient condition, treatment, and prognosis with family members and caregivers and answers their questions.
  • Support, process and facilitate end of life requests and plans within the OPWDD regulations.
  • Work with multidisciplinary treatment teams to help maintain optimal physical functioning for individuals.
  • Coordinate and supervise the treatment and care for a caseload of individuals.
  • Develop and implement training programs for staff.
  • Participate in Meetings and training as appropriate.
  • Collaborate with Nurse practitioners in the appropriate care of disabled individuals.
  • Other duties as assigned.
