Location: Jefferson Heights

Position Overview

We are hiring for three full‑time Care Coordinator positions in the Mental Health Association of Columbia‑Greene Counties Inc., supporting individuals, families, and communities throughout Columbia and Greene counties.

Job Duties
  • Coordinate with service providers and health plans to secure necessary care for members.
  • Perform fieldwork (approximately 75% of the role), traveling to clients’ homes and community sites to conduct assessments and manage ongoing care.
  • Provide transportation to clients when mandatory.
  • Connect and refer clients to appropriate services that support their care plan and treatment goals, including medical/behavioral health care, patient education, and self‑help recovery and self‑management.
  • Conduct case reviews with the interdisciplinary team to monitor and evaluate client status and needs.
  • Advocate for services and assist with scheduling needed services.
  • Collaborate with treating clinicians to ensure services are delivered and that changes in medical treatment or conditions are addressed.
  • Monitor and support clients in making scheduled medical/behavioral/social service appointments.
  • Provide crisis intervention when needed.
  • Revise care plans and goals as necessary.
  • Work remotely on a flexible basis as required by program needs and supervisor approval.
Requirements
  • Bachelor’s degree in a social service or counseling discipline (social work, psychology, nursing, rehabilitation, community mental health social work, or related field) OR a Registered Nurse with a bachelor’s degree and at least two years’ experience in direct mental health services or linkage to community services.
  • Experience in direct mental health or social service work and in linking clients to community services is necessary.
  • Reliable transportation to travel to client locations and other work sites.
  • Proficiency in, or ability to learn, online technology systems.
  • Ability to work with a diverse population in urban, suburban, and rural settings and with community members from varying socioeconomic backgrounds.
  • Demonstrated commitment to diversity, equity, and inclusion through continuous development, inclusive behaviors, and proactive bias management.
  • Regular and steady attendance is required.
  • Excellent oral and written communication skills.
  • Familiarity with Greene County and knowledge of managed care is a plus.

Conditions

All offers are contingent upon successful Statewide Central Registry (SCR), fingerprint clearance, and possession of a valid New York State driver’s license with a motor vehicle record eligible for agency insurance coverage.
