Deliver Practical, Real-Life Therapy in Patients’ Homes

@Home Therapy is seeking a Certified Occupational Therapist Assistant to provide in-home services across New York City. This role focuses on helping adult patients improve their ability to safely and independently manage daily activities within their own living environments.

You’ll have the opportunity to evaluate, treat, and guide patients using personalized strategies that directly impact their day‑to‑day function.

• Design individualized therapy programs tailored to each patient's needs.
• Incorporate adaptive techniques, equipment recommendations, and safety strategies.

• Work with patients on tasks such as self‑care, home management, and mobility within the home.
• Address coordination, upper extremity function, and activity tolerance.

• Educate patients and families on techniques to improve independence and safety.
• Provide recommendations for home modifications when appropriate.

• Complete timely documentation using an electronic system.
• Ensure all services meet regulatory and clinical standards.

• Set your own availability and manage your caseload across NYC.

• Independently evaluate and treat patients in a home‑based setting.

• Administrative coordination is handled by a support team, minimizing non‑clinical tasks.

• Per‑visit rates with the ability to scale based on your preferred workload.

• Active New York State Occupational Therapist license.
• Experience with adult or geriatric populations preferred.
• Ability to work independently in a home care environment.
• Strong clinical reasoning and communication skills.
• Comfort using EMR systems.

• Prefer functional, home‑based therapy over clinic routines.
• Value independence and schedule control.
