Full-Time Social Worker – Subacute Unit

• Complete psychosocial assessments and social service documentation in accordance with state and federal regulations.
• Participate in interdisciplinary care planning and resident care plan meetings.
• Provide counseling, support, and education to residents and families.
• Coordinate discharge planning and community services.
• Assist residents and families with adjustment, coping, and transition needs.
• Maintain accurate documentation within the Electronic Medical Record (EMR).

• Bachelor's Degree in Social Work or related human services field required;
  Master's Degree preferred.
• Minimum one year of experience in healthcare, rehabilitation, or long‑term care preferred.
• Experience with discharge planning and interdisciplinary team collaboration.
• Strong communication, organizational, and interpersonal skills.
• Proficiency with Electronic Medical Records (EMR).
