Job Summary

Provides counseling services including identifying, screening and assessing clients to determine the need for substance use treatment. Provides referrals to community-based organizations and coordinates care as needed. Responsible for facilitating therapeutic group and individual sessions. Responsible for completing and maintaining detailed OASAS-compliant case records.

Duties and Responsibilities
  • Conduct intake interviews, biopsychosocials, mental health screenings
  • Formulate treatment plans and treatment plan reviews, act as participant’s case manager to coordinate care within Crossroads and will external providers
  • Perform individual counseling duties which include holding weekly individual sessions with clients, providing referrals to community-based resources (healthcare, social services, and administrative e.g. help obtaining insurance), and developing client schedules
  • Initiate and maintain client records in accordance with policies and procedures; ensure accurate and timely data entry of all client information into the agency’s database; utilize electronic health record system
  • Facilitate assigned treatment groups
  • Perform drug testing as needed
  • Prepare monthly updates for court or other supervising agencies
  • Participate in treatment team case reviews.
  • Consult with other professionals in support of client’s treatment
  • Complete discharge documentation as needed
