- Outpatient Setting
- Experience treating lifespan
- Youngest age seen 5
- Comfortable & experience with heavy case load
- 3 month contract with malpractice coverage
- Located in Suffolk County, New York
- Active New York license
- ANCC Certified
- Completed 3600 hours to prescribe solo
- Credentialed in Medicare & NY Medicaid
